Documents (MCRD) - In English
The MCRD
is a two sided information sheet that, on the front side, provides
a list of the most common County administered programs/services
and the documents needed to apply for those programs, and on
the reverse side, provides website information and toll-free
phone numbers where documents needed to apply can be obtained.
The MCRD is available in English and the following languages:
Spanish, Armenian, Cambodian, Russian, Chinese, Korean and Vietnamese.

Face Sheet (UFS) - For an Individual
The UFS, currently being piloted, contains identifying core customer/family
history and factual information that is required by most health
and human services providers to complete the intake process. The
UFS has been designed to: create a common service application
tool across departments/agencies; establish a uniform process
for gathering needed information at the first point of contact;
streamline the referral process across agencies; serve as a universal
top sheet document in County health and human services client
records; and present an opportunity to contribute to the establishment
of requirements for an automated eligibility determination application.
