• Accessing Health and Human Services Programs – Most Commonly Requested Documents (MCRD) - In English

  The MCRD is a two sided information sheet that, on the front side, provides a list of the most common County administered programs/services and the documents needed to apply for those programs, and on the reverse side, provides website information and toll-free phone numbers where documents needed to apply can be obtained. The MCRD is available in English and the following languages: Spanish, Armenian, Cambodian, Russian, Chinese, Korean and Vietnamese.

• Universal Face Sheet (UFS) - For an Individual
  
  The UFS, currently being piloted, contains identifying core customer/family history and factual information that is required by most health and human services providers to complete the intake process. The UFS has been designed to: create a common service application tool across departments/agencies; establish a uniform process for gathering needed information at the first point of contact; streamline the referral process across agencies; serve as a universal top sheet document in County health and human services client records; and present an opportunity to contribute to the establishment of requirements for an automated eligibility determination application.

  Explains how the County of Los Angeles can help its employees manage work and family obligations.

• Understanding and Partnering with Family Resource Centers
  
  Assists County departments and agencies to better understand Family Resource Centers (FRCs) and the family support principles that guide them and to identify ways to begin partnering with existing FRCs.
  
• Planning Process for the Co-location of Multi-Agency Health and Human Service Programs
  
  Assists County departments in successfully acquiring space for new and expanded health and human service programs, many of which involve the co-location of staff and programs from multiple departments.
